Reach Out of Montgomery County is a non-profit safety net organization delivering quality medical care to the uninsured in our community. Reach Out offers comprehensive medical services including multiple evening clinics, access to a charitable pharmacy and case management into primary care medical homes. Reach Out partners with over 8 area universites and colleges to train students in community health. The clinics are staffed with volunteer physicians, nurses, pharmacists, translators, phlebotomists and medical students supported by community clerical help. Nurses are expected to effectively communicate patient condition/chief complaint as they present to the Reach Out Clinic. (The medical record reflects a detailed history obtained through the interview/triage process. Documentation will include an accurate and concise record of all services provided, treatments performed, patient education response and feedback, medication review and instructions for referral, if applicable.) Volunteer nurses are also expected to collaborate with Reach Out staff to provide resources available to Reach Out patients to promote education and understanding of disease process; encourage follow up with primary care physician, advocate health prevention; encourage participation in health fairs, screenings and attendance of other community health educational topics.
Current OH nursing license; Current CPR certification, at least one year experience
